Your health summary report is a great way to let your doctor see how you’re doing and spark more meaningful conversations. It includes averages, trends, and a summary of your readings over the last 30 and 90 days.
You can share your health summary report with your doctor, friends, or family via email, fax, or text message through your online account. You can also export and print or view your entire logbook. Just follow the directions below.
Here's how to access your health summary report:
Livongo web:
- Sign into your account
- Go to "Quick Links" Select "Sharable Data Reports"
- Select “View,” “Download,” or “Share” for the report you'd like to access
- If you choose "Share", select the method with which you would like to share — phone number, email, or fax — and select “Share.”
Livongo app:
- Sign into your account
- Select the menu in the upper right-hand corner and go to “Shareable Data Reports"
- Select the Health Summary Report
- Select "Export" and enter email address
- Select "Send to this address."
- Select "Done."
Teladoc Health web:
- Log into your Teladoc Health account
- Select "Condition Management" or go to "Programs" and then "Condition Management."
- Go to "Quick Links" and select "Shareable Data Reports."
- Select “View,” “Download,” or “Share” for the report you'd like to access
- If you choose "Share," select the method with which you would like to share — phone number, email, or fax — and select “Share."
Teladoc Health app:
- Sign into your Teladoc Health account .
- Select "Condition Management" or go to "Programs" and then "Condition Management."
- Select the menu in the upper right-hand corner and go to “Shareable Data Reports.”
- Select the Health Summary Report.
- Select "Export" and enter email address.
- Select "Send to this address."
- Select "Done."
