Blue Cross Blue Shield Michigan: How to Share Your Health Summary Report with Your Doctor or Care Provider

Attention Blue Cross Blue Shield Michigan members: If you need coverage for a GLP-1 for weight loss, there are new prior authorization criteria from BCBSM that you must meet. How to verify your Teladoc Health coaching session:

One of the BCBSM criteria is that you must meet with a Teladoc Health coach at least once. To verify that you have met the coaching requirement, BCBSM requires that you send your Health Summary Report (from your Teladoc Health account) to the in-person doctor or care provider who prescribes your GLP-1 for weight loss. You can find this report in your Teladoc Health account.

Your personal Health Summary Report is a great way to let your doctor or care provider see how you’re doing and spark more meaningful conversations. It includes averages, trends, coaching visits and a summary of your readings over the last 30 and 90 days.  

You can share your Health Summary Report with your doctor or care provider, via email, fax, or text message through your online account. You can also export and print or view your entire logbook. Here's how:

Share Your Health Summary Report from Your Online Account 

Teladoc Health web:

1. Log into your  Teladoc Health account 
2. Select "Condition Management" or go to "Programs" and then "Condition Management"
3. Go to "Quick Links" Select "Shareable Data Reports" 
4. Select “View,” “Download,” or “Share” for the report you'd like to access
5. If you choose "Share" select the method with which you would like to share — phone number, email, or fax — and select “Share"

Teladoc Health app:

1. Sign into your  Teladoc Health account 
2. Select "Condition Management" or go to "Programs" and then "Condition Management"
3. Select the menu in the upper right-hand corner and go to “Shareable Data Reports”
4. Select the Health Summary Report
5. Select "Export",  and enter email address 
6. Select "Send to this address" 
7. Select "Done"
