Who is eligible to enroll in the Diabetes Prevention Program?
This program is for people who want to live healthier, happier lives. Employees, their spouses, and their dependents are eligible as long as the employee works for one of our partner companies and has the company-provided insurance as their primary insurance provider.
Why am I being offered the Diabetes Prevention Program?
This program is offered to you as an additional health benefit by your employer. Your employer recognizes that eating well, getting active, managing stress, and getting the rest you need are important parts of preventing diabetes and staying healthy. The Healthy Living and Diabetes Prevention Program will provide you with the resources you need to manage your overall well-being.
What is the Diabetes Prevention Program?
The Diabetes Prevention Program is designed to help you prevent or prolong the onset of Type 2 Diabetes and other chronic conditions by providing you with the tools you need to set realistic goals, eat healthy, stay active, manage stress and sleep better.
There are 4 main components to the Diabetes Prevention Program.
1. Weighing in
Your connected scale automatically uploads readings to your account. We recommend that you weigh yourself with the scale at least four times a week. Weighing in frequently is the best way to see trends over time. It’s best to weigh first thing in the morning after using the rest room and before eating or drinking anything.
2. Completing lessons
Our library of lessons are delivered through the mobile app. They share the skills you need to eat healthier, exercise smarter, sleep better and stress less. They each take about 5-10 minutes to complete. You can start right after your first weigh-in.
3. Tracking food
Track your food using the “Food’ feature in the app. Keeping track of what you eat raises your awareness of your eating patterns. It can help you learn from the choices you make. You can also better monitor your progress toward a goal. And if you’re working with a coach, you’ll get more personalized tips during your sessions.
4. Connecting with a coach
Keep yourself on track by connecting with a certified coach. You can schedule a 1:1 session or send a message at no extra cost to you. Sessions are unique for each member and talk about topics important to you!
What should I expect after enrollment?
Once enrolled, you’ll receive a welcome kit in the mail within 3-5 business days.
The kit contains:
- A connected scale
- 2 AA batteries
Download the Teladoc Health app to your mobile phone from the Google Play store (for Android) or the App Store (for iOS). When your kit arrives, you will be guided through how to set up your new scale and connect it to your app. Weighing in with your new scale will unlock the rest of the program for you.
How much does the program cost?
The program is offered at no cost to you through a partnership with Teladoc Health and your employer or healthcare provider.
